2.  Fill out the online application form and click submit. The information about you and your clinic will be included on your profile in the HAMA Acupuncturist Directory. 

 3.  Payment may be made by check, credit card or PayPal.

4.  You will receive a notification by email when your application is approved. Your profile will automatically appear in the Acupuncturist Directory. You may easily add additional information such as a photo and an introductory bio by signing in to the HAMA website first, and then making your additions. You may update your profile at your convenience.
